Bongard, MD, & Darryl Y. Sue, MD DISORDERS OF FLUID VOLUME In normal persons, 50-60% of total body weight is made up of water distributed between the intracellular and extracellular spaces. Critical illness can not only result from abnormalities in the amount and distribution of water but can also cause strikingly abnormal disorders of water and solutes. Distribution of Body Water Total body water is distributed freely throughout the body except for a very few areas in which movement of water is limited (eg, parts of the renal tubules and collecting ducts).

Furthermore, some investigators have suspected that leakage of colloid into the interstitial space of the lungs and other organs can contribute to persistent organ system dysfunction and edematous states. In hypovolemia associated with ascites, rapid movement of colloid into the ascitic fluid may occur, resulting in only a transient increase in intravascular volume. In patients with nephrotic syndrome or protein-losing enteropathies, albumin and other colloids may be lost fairly rapidly. A large review of studies in which albumin solutions were given for hypovolemia, burns, or hypoalbuminemia did not find any benefit, and there was a trend toward increased mortality for patients given albumin.
